Read, fill out and print the forms so that they can be submitted to the VA, the State of Wisconsin or Edgewood College for processing. VA 24-0296, Direct Deposit Enrollment (Complete once)
VA 22-1990, Application for VA Educational Benefits (Fill out once or denied)
VA 22-1990t, Application for Individualized Tutorial Assistance (Apply once)
VA 22-1995, Request for Change of Program or Place of Training (Complete if changing programs or Transferring to Edgewood College)
