Educational Background Profile
Note: Applicants must complete Columns 1-9 to be considered for Admission.
Instructions:
| Column 1: |
Write the appropriate calendar years for every school year you attended. If you were out of school for a length of time, it must be noted. Allow one line for each year you were in school. |
| Column 2:

Write your age for each of the calendar year. If you were six years old when you attended school for the first time, write 6 on the first line. Continue by writing your correct age for each grade attended. |
| Column 3: |
For each year, write the actual number of years (not grade level) of school you attended. Regardless of what level you were i when you first attended school, that first year will be number 1. Number the next year of school attendance as 2, and number each successive school year appropriately. |
| Column 4: |
For each school year, enter the standard grade, form, class or whatever the name may be of the level you attended that year. |
| Column 5: |
Write the kind of school you attended, such as Kindergarten, Elementary School, Grammar School, Gymnasium, Lyceum, Trade School, High School, Collegio, Teachers' College, University, etc. |
| Column 6: | Write the name of each school attended. |
| Column 7: |
Write the names of the city, town or village and the country where each school you attended is located. |
| Column 8: | Specify the language used in class by your teachers. |
| Column 9: |
Write the name of any examination you passed or certificate you obtained at the end of that school year. For example, if you completed high school at the end of your 12th year in school, on that line write GCE, Bachillerato, Baccalaureate, etc. (Certified translations into English of supporting documents from the secondary level and above must accompany this form). |
