Prospective Student Checklist
- Complete the Edgewood College Admission Application.
- Submit Military Transcripts.
- Complete a Free Application for Federal Student Aid.
- Submit the following to the Veterans Benefits Team - (fax: 608-535-8322 / drop off: Office of Military & Veterans Services, DeRicci 122).
- A copy of your VA Certificate of Eligibility.
- If you have yet to apply for benefits, register for benefits with VA.gov (preferred method) by submitting a VA form 22-1990 (Application for VA Education Benefits) or VA form 22-5490 (Dependent’s Application for VA Education Benefits) electronically.
- Include any pertinent documentation that may expedite processing of your claim.
- Previous benefit recipients must complete VA Form 22-1995 (Request for Change of Program or Place of Training) or VA Form 22-5495 (Dependent Request for Change of Program or Place of Training).
- If you have yet to apply for benefits, register for benefits with VA.gov (preferred method) by submitting a VA form 22-1990 (Application for VA Education Benefits) or VA form 22-5490 (Dependent’s Application for VA Education Benefits) electronically.
- A copy of your VA Certificate of Eligibility.
- Submit the Edgewood College Veterans Worksheet.
