Disability and Accessibility Forms and Resources
How to Apply for Accommodations
Disability and Accessibility Services (DAS) works with students through an individualized, interactive process to determine eligibility for disability-related accommodations. If you have a disability, think you may have a disability, or are unsure whether accommodations may be appropriate, we encourage you to connect with us.
Getting started involves three steps:
Step 1: Complete the Disability Intake Form
Start by completing our Disability Intake Form. The form gives you an opportunity to tell us about your disability or health condition, the barriers you are experiencing, accommodations you may have used previously, and any accommodations you are interested in exploring. You also have the option to upload documentation with the Disability Intake Form, but it is not required upon submission.
Not sure what accommodations to request? That’s okay. You don’t need to have everything figured out before submitting the form. DAS can help you explore possible accommodations during your intake appointment (see step 3).
Step 2: Submit Disability Documentation
Documentation helps DAS understand your disability, its functional impact, and the disability-related barriers you experience. Documentation may include a DAS Disability Assessment Form completed by a qualified provider, a diagnostic or evaluation report, an IEP or 504 Plan, or other relevant records.
Documentation requirements vary depending on your disability and the accommodations you are requesting. If you do not currently have documentation or aren’t sure whether the documentation you have is sufficient, you can still begin the process. Contact DAS before obtaining a new evaluation or paying for additional documentation as we can help you determine what information may be needed.
Step 3: Schedule an Intake Appointment
After submitting your intake form, a DAS staff member will contact you via email to schedule an intake appointment. The intake appointment is scheduled for an hour and provides an opportunity to talk about your experiences, the barriers you encounter, accommodations that have been helpful in the past, and your current access needs. Together, you and DAS will discuss potential accommodations and determine appropriate next steps.
You do not need to prepare for the meeting or know exactly what accommodations you need before the intake. Your Disability Advisor at DAS will take lead on guiding this conversation.
What Happens Next?
After the intake process, DAS will determine your eligibility for accommodations based on the information available and your individual circumstances. If accommodations are approved, DAS will explain how to access and use them, including how to communicate approved accommodations to your instructors during an accommodation training appointment.
If additional information is needed before a decision can be made, DAS will let you know what information would be helpful and work with you to identify next steps.
